Wheelchair seating and mobility evaluations

Our wheelchair seating and mobility evaluations are for patients requiring the use of wheeled mobility in their home and work environments. We want to help you:

  • Maximize independence and safety
  • Improve posture and function in the wheelchair
  • Optimize seating comfort and function
  • Prevent complications and skin problems caused by improper seating
  • Choose a chair that fits you and your budget

A physical therapist will evaluate your mobility, overall range of motion, strength and posture. The evaluation is sent to the appropriate insurance company, equipment vendor and physician.

We take into consideration the wheelchair's cost; your insurance benefits; its size in relation to your home and work environment; types of controls you need; features such as reclining back or leg rests, upholstery, cushions and seating additions; wheel arrangements; and ongoing maintenance requirements.

If you find yourself in need of a wheeled device for the first time, or are looking for a change or upgrade for your current chair, ask your doctor to refer you to Salem Health Rehabilitation Center.

Getting a new wheelchair (the process from beginning to end)

Often insurance requires a face-to-face appointment with a physician for the sole purpose of getting a new wheelchair.

At Salem Health, evaluations will range from 60 to 90 minutes and may require a follow-up appointment.

Come to your appointment with :

  • Your current wheelchair if possible
  • Caregiver or family member as appropriate

The therapist will evaluate and document the medical need for the equipment.

The doctor reviews and signs the paperwork if in agreement.

In many cases, the wheelchair vendor will also need to perform a home evaluation to document that the equipment you choose will be able to be used in your home. Download a list of area medical equipment dealers.

The vendor will submit the paperwork to the insurance company for approval.

Once approved, the vendor will order the equipment.

The vendor will then schedule a date for delivery to your home.
