Welcome to Salem Health rehabilitation

The rehabilitation team at Salem Health provides comprehensive treatment for patients of all ages throughout the mid-Willamette Valley. Whether you've experienced an injury, are recovering after surgery or suffer from a movement or function disorder, we are here to help with multiple convenient locations in Dallas, Monmouth, Salem and Woodburn.

What to bring to your appointment

Here's some helpful things to know and bring with you to ensure a smooth and productive physical therapy session.


Wear: Comfortable, loose-fitting clothing and closed-toe, athletic shoes 

Bring: Insurance card, photo ID, relevant medical history, imaging results (X-rays, MRIs, etc.), questions and therapy goals

We ask that you please arrive 10 to 15 minutes early to your appointment. This allows time to complete any paperwork (if required) and mentally prepare to discuss symptoms and progress.

At the Salem Health therapy and community play area, Let's All Play Place, people of all abilities can come together in a safe environment that welcomes everyone.

The wheelchair-friendly play area enhances physical, sensory, emotional, behavioral and social skills for children and adults — including patients of the Salem Health Rehabilitation Center.

Pediatric rehabilitation

Salem Health’s approach to helping children with special needs — whether physical, mental or behavioral — is to maximize the child’s ability to learn, grow and play. Our pediatric therapy area helps kids with conditions such as autism, brain injury, cerebral palsy and Down syndrome, improve function safely… and still having fun!  

Our occupational, speech and physical therapists have years of experience working with children.
