Over the years, this service has grown significantly in volume and complexity, making it more challenging to manage. Because a Meds to Beds prescription requires considerably more time and coordination than a standard retail fill, we need to focus this program on patients who benefit most, such as those with transportation or financial barriers, those requiring high-risk medication teaching and hospice patients who need medication coordination before leaving the hospital.
Beginning mid-November, RNs and providers will no longer be able to order Meds to Beds through the Admission Navigator. Instead, any patient who may require this service should be coordinated directly with Pharmacy, who will screen for eligibility and schedule the patient as space allows. Due to limited capacity, not all patients will qualify, so we ask teams to use clinical judgment and refer only those who meet the need-based criteria. We also want to clarify that Meds to Beds is not a revenue-generating service — its value is in improving medication access, adherence and patient safety.
