A meniscus tear is a common knee injury where cartilage inside the joint gets ripped or damaged.
Daniel Elkin, MD, is an orthopedic surgeon at Salem Health Orthopedics specializing in the knee and shoulder. Dr. Elkin said a common question he gets from patients facing this injury is, “How long will it take to heal?”
The orthopedic surgeon said it depends on the type and location of the tear.
Factors behind recovery
“Unfortunately, many meniscus tears have a poor chance of healing on their own, because much of the meniscus has limited blood supply,” said Dr. Elkin. “Tears near the outer edge of the meniscus have a better blood supply and a greater potential to heal. Tears farther toward the center generally have much less ability to heal.”
Dr. Elkin said other factors also matter, including the pattern of the tear and whether it occurs with another injury.
“For example, meniscus tears repaired at the time of ACL reconstruction may have a more favorable healing environment,” he said.
The sports medicine surgeon said one of the most confusing parts of meniscus tears is symptoms can improve even when the tissue itself has not healed.
“Some patients become pain-free with activity modification, physical therapy and time,” said Dr. Elkin. “This can be a successful outcome, particularly for stable tears that aren't causing mechanical symptoms. However, improvement in pain does not necessarily mean the meniscus has returned to normal structurally.”
When is surgery needed?
Dr. Elkin said some patients may need surgery if they’re facing persistent pain, recurrent swelling, or an unstable tear.
“When a meniscus tear is repairable, the goal is to preserve the meniscus rather than remove the damaged tissue,” he said. “The biological healing process generally takes several months.”
The orthopedic surgeon said patients are usually able to bear weight by 4-6 weeks, return to normal activities at 2-4 months and introduce high-impact activities like running by 4-6 months.
“There isn't one healing timeline for every meniscus tear,” said Dr. Elkin. “Some tears can heal, some can become asymptomatic without healing, and others have very little chance of healing without surgery.”
