Right to Inspect and Copy
You have the right to inspect and/or receive a copy of medical information that may be used to make decisions about your health care, with certain exceptions. Usually, this includes medical and billing records, but does not include psychotherapy notes. To the extent that Salem Health maintains an electronic copy, you may request a copy of your records in an electronic format.
To inspect and copy medical information that may be used to make decisions about you, you must submit a request in writing. If you request a copy of the information, Salem Health may charge a fee for the providing the copies.
