Laboratory tests missed

25 Jan 2026

Monitoring process now in place, watch for communication

By: Andrew Judd, MD, Laboratory Medical Director; Jamie Rouse, Laboratory Services Director; and Isaura Rodriguez-Snowden, Salem Health Laboratory – State Street Manager

This story was updated 5:05 p.m. Monday, Jan. 26.

As part of routine quality monitoring, Salem Health Laboratories has identified an issue in our system where a variety of testing did not get completed for both in-house and reference lab testing. We have calculated that about 214 tests sent to Salem Health Laboratories were affected between February 2023 and October 2025.

The lab will communicate directly to those providers whose patients were missing a test by the end of the month.

We determined there was a lack of new shared knowledge on some of the capabilities within our laboratory information system. Moving forward, we are starting a monitoring process to prevent this from happening.

Salem Health Laboratories is committed to providing quality results for your patients.
