Information Prescription Form

Registered Nurses availability

10:00 AM to 7:00 PM Monday - Thursday
10:00 AM to 6:00 PM on Friday

Registered nurses are available to assist patients with their health information needs and provide them with a personalized educational experience.

The service is offered free of charge at the Community Health Education Center (CHEC) located on the first floor of building D at Salem Hospital.

Use the Information Prescription Pad to refer your patients at the CHEC. Information Prescription tear-off pads are available upon request from the CHEC.
