vteNew VTE risk assessment tool

Join us to make Salem Hospital a VTE free zone through a project commissioned by the QOC and with the full support of the MEC. On August 25, the new Caprini based risk-stratification tool and evidence-based linked orders will be required to be completed on all adult inpatients. If not completed within four hours of admission, a BPA (best practice alert ) will appear upon opening the chart. The completed risk score will be available at all points of care in Epic Navigators to provide a basis for decision making regarding VTE prophylaxis.

Please go to HealthStream for an optional CBT to learn more. On August 11 an Epic Radar message will go live and you can quick link to the tip sheets. If you have any questions, please contact John Hannig, MD, and Preethi Prakash, MD, the VTE Prevention Team Physician Champions.
