Did you know? Pharmacy tip about meds

By Matt Tanner, Lead Pharmacist

If the ‘Prior to Admission Medication List’ status (at the bottom of the Med List) is complete and either “Verbal” or “Written” (rather than “Clinic List”, “Kaiser List” or “Care Home”), the meds listed are what the patient states they are taking, even if they differ from what’s listed on the clinic or pharmacy list.

This tip helps you see the medication regimen that may have contributed to the patient’s current condition. This may, in fact, differ from what the patient’s doctor thinks he or she is taking.

For more information about the Med History Techs and their processes, call or email Matt Tanner (503-814-2048).
