Schedule first dose
The time you select is for the start of your appointment window.
When you arrive, you will be grouped with others who share the same window. There will be a wait time for your vaccination within that group.
you are unable to schedule an appointment, limited drop-in appointments are available. However, know that wait times and vaccine availability can vary for unscheduled visits.
Step 2 of 3: Report to Western Oregon University
Depending on how long the line gets, you may need to wait outside. Please dress for the weather. Be sure to wear a shirt that gives access to your upper arm — and don't forget to wear your mask.
If you have insurance, bring your insurance cards, as insurance will be billed for the administration of the vaccine. Even if you don’t have insurance, there is no cost to you.

Coming from Dallas: Drive to highway 99W and head south. Take a right on Hoffman. Turn left onto Monmouth Avenue. Right onto Stadium drive. Left on Jackson where you will find the screening station in the parking lot south of
the WOU football field.
Coming into Monmouth on 99W from the south: Turn left onto Main Street at the four-way signal. Follow Main Street until you come to the stop sign at Monmouth Ave. Take a right onto Monmouth Avenue. Left on Jackson where you will find
the screening station in the parking lot south of the WOU football field.
Step 3 of 3: Get your second dose
A second dose will be scheduled for you automatically when you receive the first dose.
If you need to cancel or reschedule:
Log in to your MyChart account and click on “My Visits” to see your appointment and reschedule. Note: Appointments for second doses must be on or after your due date: 21 (Pfizer) or 28 (Moderna) days after your first dose.
If you did not set up a MyChart account before your first dose, you still can. After creating an account, follow the instructions above to reschedule your second dose.
If you do not wish to set up a MyChart account, you may drop into the vaccine clinic where you received your first vaccine on or after your second dose due date. Be sure to bring the vaccine card you received with your first dose.
Returning for a second dose is your responsibility — we will not send you reminders if you do not have a MyChart account. Drop-in availability is limited, and we cannot guarantee that we will have a vaccine for you if you do not have
an appointment. Check our website for the hours of each location as they do vary week to week.
Salem Health intends to continue operating vaccine clinics until all of the state of Oregon is vaccinated. Plan on returning to the same location for your second dose unless Salem Health announces a change.
