Hours of operation

Salem Hospital main campus outpatient imaging (including mammography)

Monday – Friday, 7 a.m. to 6:30 p.m.
Saturday – Sunday, hours vary by department, call for availability

Late evening and weekend appointments are occasionally available. To inquire about appointment availability, please call scheduling at 503-814-5293 (Monday through Friday, 7:30 a.m. to 5:00 p.m.).

West Valley Hospital outpatient imaging

Monday – Friday, 7 a.m. to 6 p.m.

Salem Health Urgent Care imaging (for Urgent Care patients only)

7 days a week, 8 a.m. to 10 p.m.

How to schedule an appointment

Patients may schedule an appointment directly, but the scheduling department must have the completed physician order form on-hand. Your physician's office can also call to schedule appointments. 

Please provide 24-hour notice if you need to change your appointment so we can accommodate other patients.

Scheduling tips

Here are a few helpful hints that will help streamline the check-in process when you have an imaging appointment.

Before you schedule

Schedule your exam several days before your follow-up appointment. This allows time for your doctor to receive and analyze exam results.

Please have the following information available when you call:

  • Name, date of birth and Social Security number
  • Emergency contact information
  • Insurance card
  • Name of the ordering doctor
  • Type of exam
  • Reason for the exam

Some of our check-in forms are available online if you prefer to fill them out ahead of time. Doing so can cut your check-in time in half.

The day of your exam

Please follow all the necessary preparations for your procedure.

When you arrive you will need:

  • Doctor's referral (if we did not receive it directly from your physician)
  • State-issued ID or driver's license
  • Insurance card
  • Social Security number
  • Employer's name, address and phone number

In case of a workplace injury be prepared to provide:

  • Date of the injury
  • Employer's insurance company name, address and phone
  • Claim number
  • Case manager's name

In case of an auto accident be prepared to provide:

  • Date of the accident
  • Name of the insured driver
  • Driver's insurance company name, address and phone number
  • Claim number

In a hospital setting, there are occasionally emergencies that arise that could possibly delay your appointment.  It is our expectation that you should not have to wait more than 5 minutes past your scheduled appointment time. 

Our commitment is to keep you informed of anything happening at the hospital that may contribute to an unexpected delay of more than 5 minutes.
