What to expect
Thank you for choosing Salem Health's medical clinics. We look forward to getting to know you! Here are some tips for planning your visit with us.
Please arrive 15 minutes before your scheduled appointment time. You can expect to spend 60 to 75 minutes in our office as we learn about your health concerns and goals. Be prepared to review and discuss your medical history, current health status, medications and supplements with your doctor.
Be sure to bring:
All bottles and packages for medication and supplements you are taking.
Your current insurance card and government-issued photo identification.
A list of questions or concerns you would like to discuss. (Click here to download a worksheet where you can write down your questions.)
With the exception of your first visit (when you should arrive 15 minutes early), please arrive five to 10 minutes early at each visit to complete any necessary paperwork. If you arrive after your scheduled appointment time, we will make every effort to see you. However, it may be necessary to reschedule your appointment. Please notify us at least 24 hours in advance when cancelling an appointment.
Diagnostic testing and referrals
If you need to see a specialist, your family medicine health care team will make the referral and help coordinate the transfer of necessary medical information. We will continue to manage your overall care and work with the specialist to make sure your health care needs are being met.
Medication safety is one of our chief concerns. Our providers carefully review prescription refills and may want to speak to you before authorizing a refill. Read more about Salem Health’s prescription medication policies.
We’re here when you need us
You can call us anytime, 24 hours a day, seven days a week. During our regular office hours, members of our clinical support team are available to schedule appointments or help connect you with another member of your health care team. For urgent issues after our normal business hours, our on-call providers will assist you over the phone.
Manage your health care online
You can also manage your health care any time by accessing our online patient record portal, MyChart. Ask for your activation code at your first visit so you can request appointments, track immunizations and medications, see lab and test results and email your provider with questions or concerns that don't need an immediate answer.
Payment for services
We accept most insurance plans. Copayments, coinsurance and deductibles are required to be paid at the time of service. For those who prefer self-pay, we offer a discount when payment is made in full at the time of service. Billing for laboratory, radiology and other testing services will be made through Salem Health or the outside provider of your choice.
