It is our policy at Salem Hospital to keep your health information confidential.
Medical providers are required by law to maintain the privacy of your medical information. We are required to notify you of our legal duties and privacy practices regarding your medical information, and abide by those practices described in the Joint Notice of Privacy Practices form that you receive upon admission.
We are not able to give information to your family without your permission. If you would like information shared with your family or friends, you may give them a special PIN (personal identification number). The PIN will be requested by staff if family or friends call for an update.
Medical records
To review a copy of your medical record, please download and submit the “Authorization for Use or Disclosure of Protected Health Information.” Requests must be made in writing.
