What is Condition Help Response?

Because we are committed to providing the best care to our patients, Salem Hospital participates in the Condition Help Response program.

This program was created to promote enhanced communication and patient safety in the event that a patient's condition changes and support is required.

A Condition Help Response call will initiate a team response when a patient is in need.

When to Call

  • If there is a noticeable change in the patient's medical condition, first contact the patient's nurse.
  • If you feel after speaking with a member of the healthcare team that additional support is needed, request Condition Help Response.

How to Access

Condition Help Response Call 2-3456 from any hospital phone. The operator will ask you to identify yourself, your location (such as room number) and patient name.

The Condition Help Response team will be activated and arrive promptly to evaluate the patient.
