Comprehensive Pain Center

Willamette Health Partners has made a business decision to reevaluate our pain management specialty practice and will be pausing operations of the Comprehensive Pain Center on Oct.11, 2013.

The pain center will focus on completing care of existing patients and will not be accepting new patients after Aug. 19, 2013.

To find a new pain management provider, you may contact:

  • Your primary care provider.
  • Your health insurance plan.
  • Marion-Polk Medical Society at 503-362-9669 or online.
  • American Medical Association at 800-262-3211 or use their online Doctor Finder.

We apologize for any inconvenience this causes. Comprehensive Pain Center providers and team members wish you the best in your future health. 

Current and returning patients

We will transfer your medical records to whomever you designate as your new provider.

Once you have selected a new provider, please complete the Authorization to Release Medical Records and return it to the Comprehensive Pain Center by Oct. 11, 2013.

After that date, you may submit the form to the Salem Hospital Health Information Management Department on the second floor of Building B on the Salem Hospital campus or mail to the address on the form.

We will make every effort to ensure your medical records are transferred to your new provider in a timely manner.
